How do I schedule an appointment?
You can request an appointment through our website or call/text our office at (406) 595-1920. Our team will help find a time that works best for you.
Appointments are confirmed once intake forms and a card on file are completed.
What do I need to do before my first visit?
All new patients are required to complete intake forms and keep a valid credit card on file prior to their appointment.

Appointments are confirmed once intake forms are completed and a card is on file. Appointments may be released or rescheduled if these steps are not completed in advance.
What should I expect at my first visit?
Your first visit includes a full evaluation, review of your symptoms and history, and a personalized plan of care to help you reach your goals.

Where do I complete my intake forms?
Your intake forms will be sent through our secure patient portal and should be completed prior to your appointment.
What can I do in the patient portal?
You can view appointments, complete forms, make payments, and securely message your provider through the portal.
How will you contact me?
We may contact you by phone, text, email, or through the patient portal depending on the type of communication

Do you accept insurance?
We are in-network with several insurance providers, including Blue Cross Blue Shield (most plans), Allegiance (Cigna), PacificSource, Aetna, Mountain Health Co-Op, and others.
How do I know what my insurance will cover?
We are happy to provide a list of accepted insurance plans; however, patients are responsible for verifying their own coverage, benefits, and eligibility prior to their visit. Final costs are determined by your individual insurance plan.
What if I’m out of network?
You are still welcome to be seen as a self-pay patient. We can provide a superbill for possible reimbursement.
Do you offer self-pay options?
Yes, self-pay options are available for patients who prefer not to use insurance or are out of network.
Why do you require a card on file?
A card on file is required to reserve your appointment and may be used for late cancellations, missed appointments, or any remaining balance after insurance. Your card is not charged at the time of booking.

What is your cancellation policy?
We require at least 24 hours’ notice if you need to cancel or reschedule your appointment.
What happens if I cancel late or miss my appointment?
Late cancellations may incur a late cancellation fee.
Same-day cancellations and missed appointments (no-shows) are billed in full for the scheduled visit.

What types of conditions do you treat?
We specialize in pelvic health physical therapy for women and men.
Including but not limited to pregnancy and postpartum care, pelvic pain, incontinence, and orthopedic-related conditions.
Do I need a referral?
In many cases, a referral is not required. Some insurance plans may require one, and we’re happy to help you understand what your plan needs.
Please note: Medicare requires a referral to book.
What should I wear?
Comfortable, movement-friendly clothing is recommended for your evaluation and treatment.
